Be sure to check for the following health issues

※Customers who answer "yes" to the following questions will not be able to participate in our itinerary. Please confirm again before making a reservation. If you have any of the following conditions, please be sure to bring a doctor's note to participate.

  • ✔ Do you currently have an ear infection?
  • ✔ Do you have a history of ear disease, hearing impairment, or balance problems?
  • ✔Have you ever had ear or sinus surgery?
  • ✔ Do you currently have a cold, stuffy nose, sinusitis or bronchitis?
  • ✔ Do you have a history of circulatory problems, severe hay fever, allergic reactions, lung disease?
  • ✔Have you ever had a pneumothorax or chest surgery?
  • ✔ Do you have a history of active asthma, emphysema, or tuberculosis?
  • ✔ Are you currently taking any medications that carry warnings about impairing physical or mental abilities?
  • ✔ Do you have behavioral health issues, psychiatric issues, psychological issues, neurological conditions?
  • ✔ Are you pregnant or may you be pregnant?
  • ✔Have you ever had a colostomy?
  • ✔ If you have heart disease. Have you ever had a heart attack? Have you had heart surgery or vascular surgery?
  • ✔ Have you ever taken medication to control high blood pressure, angina or blood pressure?
  • ✔ Are you over 45 and have a family history of heart disease or stroke?
  • ✔ Do you have a history of bleeding or other blood disorders?
  • ✔ Do you have a history of diabetes?
  • ✔ Do you have a history of epilepsy, temporary memory loss or fainting, convulsions, or are you taking any medications to prevent these symptoms?
  • ✔ Do you have a history of back, arm or leg problems after an accident, broken bone or surgery?
  • ✔ Do you have a history of claustrophobia or agoraphobia? (Fear of closed or open spaces)

 

Age 60+ Scuba Diving

If you are over 60 years old and still want to participate in our diving activities, please use the attached form for a doctor's note. Originals must be submitted to us on the day of the event.

If you are over 65 years old and come for snorkeling activities, you can also use this form. Of course, the diving standard is higher, but it can be used as a reference for the doctor to check you.
